If you would like to submit a New Member Information form, you are in the right place!

Instructions:

  1. Download and fill out the membership form (Membership Form PDF).  Instructions for filling out the form are below. Thanks to Tae Kim for editing the form to pre-fill our information!
  2. Submit the form via mail or email
  3. We will contact you for dues collection. You’ll be able to either pay by check, or online with your credit card. Read more about Honolulu Sertoma Dues

Fill out the form with your:

  • Name
  • Mailing Address
  • Date of Birth
  • Spouse
  • Sign and Date it
